The website works best in Internet Explorer versions 6.0 and higher and Firefox versions 2.0 and higher. Users may experience issues with other browsers and are recommended to use the browsers listed above. It is recommended that browser windows be opened using the icon on the desktop to avoid shared browser sessions. Some browsers share sessions regardless of how the browser is opened. Please check with the browser's vendor about session management. When NPPES detects multiple browsers open within the same session, NPPES will terminate the session to protect the data in NPPES. Data entered will be lost and will need to be re-entered. Use of the Back and Forward browser buttons could result in loss of all the information entered. Create Login for Existing NPI Page

Important Info: User IDs cannot be changed. Personal information, such as a Social Security Number or a National Provider Identifier, should not be used as the User ID. Once you have successfully created a User ID and secret question/answer combination and submitted the record, the User ID and Secret Question/Answer combination will remain tied to your record. Please use the Next button to navigate to the next page in the application.

An enumerated provider who applies for an NPI through a paper application or EFI Organization does not have a User ID and Password that will allow him/her access through the web. In order to have access to his/her NPI Application, the provider is required to create a User ID and Password on the Create Login for Existing NPI Page.

The required information is as follows:

For an Individual: NPI, Provider First Name, Provider Last Name, SSN (Social Security Number) and Provider Date of Birth (format: MM/DD/YYYY).
A login cannot be created if a valid SSN does not exist in NPPES. Please contact the Enumerator for further assistance.

For an Organization: NPI, Organization Name and EIN (Employer Identification Number).
