The website works best in Internet Explorer versions 6.0 and higher and Firefox versions 2.0 and higher. Users may experience issues with other browsers and are recommended to use the browsers listed above. It is recommended that browser windows be opened using the icon on the desktop to avoid shared browser sessions. Some browsers share sessions regardless of how the browser is opened. Please check with the browser's vendor about session management. When NPPES detects multiple browsers open within the same session, NPPES will terminate the session to protect the data in NPPES. Data entered will be lost and will need to be re-entered. Use of the Back and Forward browser buttons could result in loss of all the information entered. Individual providers that render health care services will be required to indicate their classification name / area of specialization on this page.
1. Continue your taxonomy selection by choosing a classification name / area of specialization combination. (e.g., Allopathic and Osteopathic Physicians, Dermatology)

2. Certain taxonomy selections will require you to enter your license number and the state where the license was issued. (See the following table). Select Foreign Country in the state drop down box if the license was issued outside of United States.

The License Number field allows the following special characters: ampersand, apostrophe, colon, comma, forward slash, hyphen, left and right parentheses, period, pound sign, quotation mark, and semi-colon. A field cannot contain all special characters.

DO NOT report the Social Security Number (SSN), IRS Individual Taxpayer Identification Number (ITIN) in this section.
